Best Hospitals in the USA 2026-2027: The Honor Roll, Specialty Leaders and How to Choose

If you search for the best hospital in America, you will find a different answer on almost every page. Some lists are built on patient reviews, some on safety grades, some on what other doctors say. The one most Americans end up quoting is the U.S. News and World Report Best Hospitals ranking, and the 2026-2027 edition came out on 4 August 2026.

I want to do two things in this guide. First, lay out what the new results actually say, accurately and without the marketing gloss. Second, explain how to turn a list of famous names into a decision you can act on, because the hospital that is best for the country is rarely the hospital that is best for you.

The 2026-2027 Honor Roll

U.S. News evaluated close to 4,500 hospitals this year. Only about 16 per cent of them earned a Best Hospitals designation, and a smaller group made the Honor Roll, the list reserved for hospitals with excellent results across the full range of adult care.

Twenty hospitals made it. They are published in alphabetical order rather than ranked one to twenty, so be wary of anyone who tells you they are “the number one hospital in America” on the strength of this list.

Some patterns are worth pointing out. Mayo Clinic holds three of the twenty places through its Minnesota, Arizona and Florida campuses, which says something about how well a single organisation can reproduce its way of working in different cities. New York has three, Boston two, Chicago two and Los Angeles two. UCSF’s own announcement notes this is its 30th consecutive year on the Honor Roll, a streak that is hard to argue with.

The flip side is that the Honor Roll clusters in a handful of big metropolitan areas. If you live in Montana, Alabama or rural Pennsylvania, it is not a shopping list. It is context.

What changed in the methodology

This year’s edition shifted the weighting toward results. Reporting on the release describes about 80 per cent of the score as based on risk-adjusted outcomes, such as survival and complication rates, and about 10 per cent on patient experience, with particular emphasis on nurse communication scores from the national HCAHPS survey. Previously, outcomes, expert opinion and patient experience each carried roughly a third.

That matters because it moves the ranking away from reputation, which tends to reward institutions for being famous, and toward what actually happened to patients. Risk adjustment is the important phrase there. It means hospitals that treat sicker, more complex patients are compared with what would be expected for that mix, so they are not punished for taking on difficult cases.

U.S. News also added four new regional specialty rankings this year, covering cancer, cardiology and heart surgery, orthopaedics and rehabilitation. Regional specialty rankings are an underrated addition, because they answer the question most patients actually have: what is the best option within a reasonable distance of home?

The best hospital in each specialty

Here is where the list becomes useful. If you know your diagnosis, the specialty ranking is a much better guide than the Honor Roll. These are the hospitals ranked first in each of the 14 adult specialties for 2026-2027.

Notice how many of the specialty leaders are not general hospitals. MD Anderson and Memorial Sloan Kettering are cancer centres. Hospital for Special Surgery does orthopaedics. Bascom Palmer is an eye institute. Shirley Ryan AbilityLab is a rehabilitation hospital. These places do a very large volume of one kind of work, and volume is one of the most reliable predictors of a good result.

NYU Langone is a rare double: first in both cardiology and neurology. Mayo Clinic leads two digestive and metabolic categories. Johns Hopkins leads in gynaecology and rheumatology, and UCSF leads geriatrics and pulmonology.

How to choose, step by step

Rankings are the start of the process, not the end of it. Here is the sequence I would follow for a serious condition.

1. Get the diagnosis right first

The most expensive mistake in healthcare is treating the wrong thing well. If your diagnosis is uncertain, rare or has changed more than once, a second opinion from a major academic centre is worth considering before anything else. Many of the hospitals above offer remote reviews of your imaging and pathology for a fixed fee.

2. Look at the specialty table, then the regional one

Find your condition in the specialty rankings. Then check whether a regional specialty ranking, new this year, shows a strong hospital closer to home. For many cancers and heart conditions, you can get excellent care within driving distance, and staying near your support network has real value during treatment.

3. Ask about volume and outcomes

Phone the department and ask how many patients with your diagnosis they treat each year and how many procedures like yours the named surgeon performs. For complex surgery such as pancreas, oesophagus, major lung and aortic operations, higher volume is consistently associated with lower mortality. A good department answers this readily.

4. Check that the whole team is in network

This is where people get hurt financially. The hospital may be in your plan’s network while the anaesthesiologist or radiologist is not. Under the federal No Surprises Act you are protected from most surprise bills for emergency care and for out-of-network clinicians at in-network facilities, but that protection does not cover everything, and ground ambulance rides are a notable gap. Our guide to hospital costs and insurance walks through the details.

5. Confirm prior authorisation in writing

Even when everyone is in network, your insurer may require approval before a procedure. Get the authorisation number and the dates it covers, and make sure it matches the facility where the procedure will actually happen.

6. Think about aftercare

Complications and follow-up appointments happen after you go home. If you travel for surgery, ask how follow-up will work, who manages your medication, and which local doctor will receive your records.

Where the Honor Roll hospitals sit on the world stage

Newsweek’s World’s Best Hospitals 2026 ranking, which uses a different method, places four of the same American institutions in the global top ten: Mayo Clinic, Cleveland Clinic, Massachusetts General Hospital and Johns Hopkins. If you are curious how the global picture compares, see our breakdown of the top 10 hospitals in the world.

The two rankings agree on the usual suspects, but their methods differ, which is exactly why you should not rely on any single list. When two independent systems point to the same hospital, that is a stronger signal than either on its own.

A note on Mayo Clinic, three times over

Mayo’s triple appearance deserves a comment, because it is easy to misread. The Rochester campus is the original and the one that leads the diabetes and gastroenterology specialty tables. Arizona and Florida are separate hospitals with their own staff, but they share Mayo’s electronic records, protocols and culture of group practice. For a patient, that means a person in Phoenix or Jacksonville can reach the same approach without a trip to Minnesota. It is a model other systems have tried to copy for years, with mixed success, and the Honor Roll suggests why: consistency across sites is much harder to achieve than excellence at one.

The same applies on a smaller scale to the big academic systems in New York and Boston, where one health system may own several hospitals with quite different strengths. Always check which campus and which department you are being referred to, rather than assuming the brand name guarantees the same quality everywhere.

Limits of any hospital ranking

Be sceptical in a healthy way.

  • Averages hide individuals. A hospital’s excellent record in cardiac surgery does not guarantee that your surgeon is among its best.
  • Medicare data dominates. Much of the outcome data comes from Medicare claims, which describes patients over 65. If you are 35, the data applies less directly.
  • Rankings are marketing. Hospitals pay close attention to these lists and promote them heavily. That is not a reason to distrust them, only a reason to read the original source rather than the banner ad.
  • Access matters. A top-ranked centre with a two-month wait for a new patient may be the wrong choice for a fast-growing cancer.
  • Cost is not part of the rank. The most prestigious hospitals often negotiate the highest prices.

Frequently asked questions

What is the number one hospital in the USA?

There is no single official number one in the U.S. News 2026-2027 Honor Roll, which lists 20 hospitals alphabetically. Mayo Clinic is first in the Newsweek World’s Best Hospitals 2026 list, and different rankings use different methods.

Are Honor Roll hospitals better for everything?

No. They perform well across a broad range of adult care, but a specialty hospital such as Hospital for Special Surgery or Bascom Palmer may be the better choice for orthopaedics or eye disease.

Does a higher rank mean a higher bill?

Not necessarily, but prestigious hospitals frequently have higher negotiated rates. Always confirm network status and ask for a cost estimate before planned care.

Can I get care at an Honor Roll hospital if I do not live nearby?

Often yes, through referral, a remote second opinion or travelling for a specific procedure. Check whether your insurance covers care that far from home.

The short version

  • The 2026-2027 U.S. News Honor Roll has 20 hospitals in alphabetical order, including three Mayo Clinic campuses.
  • This year’s method puts about 80 per cent of the weight on risk-adjusted outcomes.
  • For a specific diagnosis, the specialty rankings matter more than the Honor Roll.
  • The best predictor of your result is volume: how often the team does exactly your procedure.
  • Before booking, confirm network status and prior authorisation for the facility and the surgeon.
